get started! After calling and setting up a complimentary
45 minute appointment with me, I have a few forms which you
need to carefully read through and fill out and sign.
These include the HIPPA Statement, Disclosure Statement and
Intake form. For the HIPPA form, you will
need to print two copies, sign your signature
at the bottom and date it. Bring both copies in to my office
on your initial session, so that I can sign both copies as
well. One copy you will need to retain for your records, and
one I will keep in your file. The Disclosure Statement
also requires you to print two copies. After
you have read through it, you will also need to sign, date
and bring in both copies. I will also sign both copies, returning
one to you for your records and retaining one to keep in your
file. Lastly, print one copy of the Intake
form, and fill that out, and bring it with you. This
I will keep in your file, as well.
